Your doctor will provide you with the date and arrival time of your surgery. The doctor’s office provides surgery center staff with your surgical case information. Please make sure that your doctor has your current contact information in case the surgery center needs to contact you before surgery.
You will be provided forms that you will need to complete before surgery. This allows the Surgery Center to obtain your medical and surgical history so anesthesia can be administered safely. You MUST complete these forms prior to surgery to avoid delay or rescheduling.
